Membership Suspension/Cancellation Request

The City of Armadale is collecting your personal information to process and manage your membership, booking, or enrolment, communicate with you about your related AFAC services, and perform its functions and meet its legal obligations. The information is collected where necessary to provide the requested service or where authorised or required by law. If the requested information is not provided, the City may be unable to process your membership, booking or enrolment. Your personal information may be disclosed to the City's contractors, service providers and other government agencies where authorised or necessary to deliver the requested service, where required by law or in an emergency. You may request access to or correction of your personal information, or make a privacy complaint, by contacting the City via privacy@armadale.wa.gov.au or (08) 9394 5000.
